Service Agreement (Families)
Include Pty Ltd will provide the following setup services for $500 (GST free).
Initial set up of program including:

• Initial assessment(s) with written report. Normally this will involve an Alpern Boll Developmental Profile to assess the child's developmental level.
• Workshop on theoretical basis to child development and associated issues.
• Hire of reading program teacher book for 2 months.
• Hire of teacher manual.
• Provision of first take home book.
• Up to 5 home visits in the first two months to establish the program in the home with families competent and confident of teaching reading.
• Advice on other developmental issues that arise in the period.

Additional Ongoing Charges

• $100 per calendar month for book hire.
• Assessments with report (approximately every three to six months or 40 lessons, whichever comes first). $200 per assessment. One assessment not charged if on program for 3 months or more.
• Hire of additional teaching material; $100 per month per area taught (i.e. reading, mathematics and language)
• Take home (homework) books. One needed approximately every 50-60 lessons on each program. $25 each for take home books.
• Consultant (Dr Bob Jackson): $50 per hour including travel time for any specific issues outside of the program. Travel time is calculated to and from Carmel. Telephone advice is not normally charged unless it is extensive and charges have been negotiated.

Payment agreement:

• Payment in advance, although individual arrangements can be made for the initial payment. This amount is non-refundable if a decision is made to withdraw from the program at any stage.

Can't afford this? While the costs are low compared to much less effective alternatives, we are aware that some families would struggle to cope with these charges, and many children who struggle with reading come from families with limited funds. If you contact us, we are trying to gain assistance from business to 'sponsor' children and may be able to put you on this program. You would need to be willing to enter an agreement about the teaching, have you child's name given to the business involved and copies of the results made available to them.

• Advice on other issues that arise in the period.

• Assessments with report (approximately every six months ). $295 per assessment. One assessment not charged if on program for 6 months or more.
• Hire of additional teaching material; $100 per month per area taught (i.e. reading, spelling, mathematics and language)
• Take home (homework) books. One needed approximately every 50-60 lessons on each program. Approximately $25 each for take home books (may be more for some programs).

Numeracy and Spelling

Can't spell to save yourself? Worried about your child's spelling? Even though we have computer spell checkers these days, not being able to spell can be embarrassing and reduce employment prospects. However, spelling is easy when you are taught properly. What about maths? Same thing. We can learn if taught properly.

In addition to reading, programs are available for numeracy and spelling for both children and adults. These are the same cost as literacy if taken alone, or less if taken in addition to other programs. Contact us for details.

If additional research is required for a specialised presentation, costs would be adjusted -- $100 per hour for each additional hour of preparation. Outside of Perth Metro, additional charges for travel and accommodation are likely to apply.

Small presentations (up to two hours): First hour: $300 including preparation and travel (Minimum Charge). Second hour, $150.

Half day workshops: $600 up to 4 hours including breaks.

One day workshops or multiple day workshops: $1200 per day up to 8 hours including breaks. Any additional time on any day (e.g. evening session) charged at $150 per hour.

All prices plus GST.

Workshop fees include preparation of teaching materials and provision of ONE copy of handouts, plus assistance with design of the workshop flyer, if required. Include Pty Ltd will not be responsible for arrangements outside of the presentation (e.g. venue, food, drink, advertising, registration, seating arrangements, photocopying etc) unless previously arranged at additional charge.

Requirements for presentations are:

• A quiet room with lighting subdued enough for overhead/data projections to be easily visible by the audience.
• Additional small group room(s) if required (check when booking Include Pty Ltd).
• A white board and suitable pens and eraser.
• 'Butcher's paper', Blutack and suitable pens (May not be required on all workshops -- check).
• A video/computer data projector capable of projecting at XGA and 1200 lumens or above. Appropriate VGA connecting cable. Alternatively, this can be provided at a cost of $80 per day or $20 per hour for shorter periods (plus GST).
• Screen or white wall for projecting onto.
• VHS video player and TV.
• Overhead projector for backup in case of technical failure of other equipment.
• Necessary extension cords and power board.
• Table for presenter materials/projector/computer etc.
• Refreshments for presenter(s) - (same as for course participants - e.g., tea/coffee/lunch as provided).
• Someone available to introduce presenter(s) and assist with any emergencies that might occur (e.g. power failure! -- Murphy is alive and well.)

Agencies, Schools, Business, Government: For work outside of the Perth Metropolitan area, the following arrangements and charges for Travel and Accommodation will apply. Note that time in travel will also be charged, particularly on short trips-- rate negotiated individually.

Interstate:

Travel economy class by Qantas. (Where applicable, book under the name Dr R. Jackson and quote frequent flier number 0102282 when booking flights). Any excess baggage charges incurred.

Accommodation: 4 star or above where overnight accommodation is required. Note that for all eastern States Venues, travel east from Perth will need to be on the previous day due to time differences.

Taxi fares to and from both Perth airport and destination airports (unless supplied). Any taxi costs to and from motel and venue(s). Car hire costs if previously arranged as an alternative to taxi charges.

Daily rate: $70 per day to cover meals and incidentals. For part days, breakfast $15, Lunch $15, Tea $40.

In WA:

Travel: Hire car costs or mileage on private car at government mileage rates. Plane fares if applicable. Where applicable, book flights under the name Dr R. Jackson (Please quote Qantas frequent flier number 0102282 when booking flights). Bus or train fares if applicable.

Accommodation: 4 star or above where overnight accommodation is required. Where extensive travel is involved, accommodation may be required before the event to allow preparation time.

Taxi fares to and from both Perth airport and destination airports (unless supplied). Any taxi costs to and from motel and venue(s). Taxi costs to and from bus or train.

International: Negotiated for each country. Interstate charges above would be a guide, although the daily rate would be higher for many overseas destinations.

Where costs are a significant imposition on a family, lower quality accommodation, billeting or other arrangement may be possible. However, costs incurred for travel and accommodation would have to be met in all cases.
